CAPA Process: From Finding to Prevention. Corrective action prevents recurrence of an identified nonconformity, while preventive thinking looks ahead at potential failures. A CAPA system needs clear triggers, proportionate investigations and closure criteria.

Why it matters: An action is not effective simply because somebody updated a policy or completed a training session.

First, define the scope: Record a specific nonconformity and contain any immediate risk to product, learner, customer or patient.

Next, apply the process: Investigate causes systematically; select actions that address the process and assign deadlines, resources and owners.

Then, check effectiveness: Verify implementation and later effectiveness using a metric linked to the original failure mode.

An example from professional practice: Missing certificate issue dates recur each month. A software validation rule with exception alerts may be more sustainable than reminders to individual coordinators.

Mistakes that weaken results: Conflating containment with prevention, closing without evidence or creating so many low-value CAPAs that serious issues receive little attention. Investigators and auditors should seek verifiable evidence rather than assuming written intent matches actual performance.

Records worth keeping: Finding, risk and impact review, cause analysis, approved change, implementation proof and effectiveness result. Link each record to the relevant decision or operational control so another qualified reviewer can follow it.

A practical first action: Write a closure criterion for an open CAPA before asking the owner to complete it. Record the observed gap and review whether the change works after implementation.
