Adverse Event vs Serious Adverse Event: An adverse event is an untoward medical occurrence associated with a medicinal product or study participation that does not necessarily have a causal relationship. Seriousness uses defined criteria; severity describes intensity.
Why professionals need this knowledge: Confusing seriousness with severity can delay urgent reporting or create misleading safety information.
Step 1: Document the medical event, onset, outcome, relevant history, treatments and source information without assuming causation.
Step 2: Assess seriousness using the protocol and regulatory criteria, and separately evaluate severity, relatedness and expectedness as required.
Step 3: Escalate within applicable reporting timelines and follow up missing information while preserving an audit trail.
Worked example: A participant is hospitalised for an event that is clinically mild in intensity. Depending on the circumstances and definitions, the event may still meet a seriousness criterion.
What to avoid: Calling all severe events serious, withholding reports while waiting for complete information or automatically attributing every event to the study drug. An apparently neat form is not a substitute for reliable actions and sound judgement in the real workflow.
Records and audit trail: Source notes, event narrative, chronology, assessments, reports and follow-up correspondence. Accurate records enable independent review and appropriate follow-up when concerns arise.
Try this exercise: Practice classifying three example events using separate seriousness, severity and relatedness columns. Discuss the exercise with an appropriately qualified colleague before applying it to a live case.
Standards and references: ICH clinical safety reporting guidance and applicable jurisdiction-specific safety reporting regulations. This is educational material; the approved protocol, governing standard and local law take precedence.
